HealthPoint is hiring for a full-time Diabetes Educator to enhance the lives of patients. This position will be working in the Covington and Florence locations. Responsibilities
Experience working in an ADCES Accredited Diabetes Education Program providing one-on-one diabetes and nutrition education Assist in developing core curriculum and patient materials Provide one-on-one education sessions to teach diabetic patients about diabetes and how diet impacts their condition Ensure meal plans are affordable and accessible with easy to understand recipes Monitor patient progress, adjust meal plans and education as needed, and provide ongoing support and encouragement through follow up visits Use a variety of teaching strategies and methods that consider learning preferences, reading level, readiness for change, empowerment, motivational interviewing, and local cultural values Interpret personal health data, develop individualized care plans based on assessed needs, goals and cultural beliefs, and promote self-management Maintain appropriate certifications (CDCES or BC-ADM) as part of professional requirements Qualifications
Certified Diabetes Care and Education Specialist (CDCES) or Board Certified in Advanced Diabetes Management (BC-ADM) Experience in an ADCES Accredited Diabetes Education Program Benefits
