Clinical Pharmacist - Hospital Care at Home Join the Cleveland Clinic as a Clinical Pharmacist and provide pharmacy services, ensuring appropriate medication selection, dosing, administration, and dispensing. In this part‑time role (7:00 am‑3:30 pm with periodic weekend shifts), you will support patients who are considered inpatient but reside at home, reviewing charts, verifying medications, and collaborating closely with physicians on care transitions.

Responsibilities

Provide safe, efficient, patient‑centered, evidence‑based pharmacy and medication services.

Ensure appropriate selection, dosing, administration, and dispensing of medication orders.

Verify medication orders and dispense medications accurately according to state and federal regulations.

Document services in a complete, accurate and timely manner.

Actively contribute to medication safety initiatives.

Maintain compliance with national accreditation standards.

Educate and train students, residents, colleagues, and patients.

Provide cost‑effective care.

Minimum Qualifications

PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy.

Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire.

Basic Life Support (BLS) certification through the American Heart Association (AHA).

Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations.

Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the AHA upon or within a period of 90 days of hire for individuals responding to adult or pediatric cardiopulmonary arrest events.

Preferred Qualifications

Board Certification.

Completion of a Post‑Graduate Year 1 Residency (PGY1), fellowship or relevant post‑graduate pharmacy experience.

Post‑Graduate Year 2 Residency (PGY2) or relevant experience for Clinical Pharmacists in a specialized pharmacy practice area.

Prior inpatient experience.

Physical Requirements

Ability to perform job while standing and walking for prolonged times approximately 75 % of shift.

Manual dexterity to perform computer and dispensing functions; ability to lift up to 30 lbs.

Reaching, stretching, stooping, climbing steps and sitting as needed.

Normal or corrected vision.

Clear verbal communication by phone or in person.

Personal Protective Equipment

Follow standard precautions using personal protective equipment as required.
