Job Overview
The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The role demonstrates the Sunrise Senior Living mission, and promotes the highest quality of care and services in accordance with applicable laws, Sunrise standards, policies, and procedures. Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents. Participate in the development of ISPs and monthly updates. Review designated assignments. Be responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift. Observe, report, and document symptoms and changes in condition (skin, behavior, alertness, weight, diet, participation in activities). Attend daily Cross Over meetings with the lead care manager. Notify supervisor and Resident Care Director if a resident has increased care needs. Inform supervisor of any resident changes in condition. Respond to security system and resident call bells promptly and take appropriate action, including resetting bells. Greet guests, family members, residents, and team members; answer, direct and respond to phone calls professionally and promptly. Assist with continence management and dispose of continence products properly to ensure sanitation of resident suites and restrooms. Communicate with families as needed and act as a resource. Help residents maintain independence and safety while promoting dignity and adhering to Resident Rights and Sunrise Principles of Service. Assist with activities of daily living (ADLs) and encourage participation in ISP-directed activities. Engage residents in life skills and life enrichment activities throughout the day in reminiscence. Demonstrate empathy, consider each resident’s communication patterns or history, and meet basic human needs. Maintain a warm, calm environment and promote safety and comfort; follow established safety regulations and infection control practices. Host and engage in daily activities with residents. Risk Management and General Safety
Collaborate with the community team to ensure compliance with health and safety regulations and risk management programs. Follow safety procedures, including use of PPE, fire safety, SDS, and lockout/tagout when applicable. Report all accidents/incidents and unsafe conditions immediately. Ensure cords, carts, equipment, and hazards do not obstruct exits and comply with fire codes. Understand and follow proper procedures for exposure to bloodborne pathogens and hazardous materials; report exposures as required. Ensure oxygen tanks are stored safely and monitored as prescribed. Housekeeping and Laundry Services
Maintain and clean resident rooms as assigned, including beds, trash, clothing, and floors. Wash resident laundry as directed by the ISP; manage dining room linens and napkins. Complete assigned housekeeping tasks and keep common areas clean and tidy. Dining Service
Serve meals in the dining room and assist with dining room operations as assigned. Promote a pleasant dining experience and follow residents’ diets per ISP/Confidential Diet Board. Observe and document changes in dining habits; respond to dining needs while upholding Sunrise hospitality standards. Provide room service delivery as needed and practice safe food handling. Specific Responsibilities for the Reminiscence Neighborhood Care Manager
Encourage residents in life skills and enrichment activities as indicated on ISP and demographic profile. Support attendees at afternoon social and integrate life skills into daily routines. Incorporate the concept of Creating Pleasant Days and multi-sensory experiences into residents’ daily routines. Engage in monthly letters home with residents and families. Training and Team Contributions
Participate as a team member and work toward team goals; uphold the Team Member Credo and Principles of Service. Contribute to engagement programs and participate in surveys and improvement workshops. Attend regular meetings and trainings; comply with infection control and regulatory training requirements. Perform other duties as assigned and maintain Sunrise standards. Core Competencies
