Overview

Full-time Physical Therapist Case Manager — Home Health. Anticipated Territory: Santa Cruz County (Watsonville and surrounding cities). Requires being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless noted. Base pay range: $120,000.00/yr - $135,000.00/yr. Additional compensation opportunities include bonuses as described below. What you’ll do as a Home Health Physical Therapist Case Manager

Provide skilled services to evaluate and address the needs of individuals in their homes to help them live safely and productively at home. Restore function to minimize disability following disease or injury and help patients reach maximum performance within their capabilities. Evaluate and treat patients using medically prescribed physical therapy programs. Supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Develop and implement detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patients’ progress to the treating MD. Qualifications and requirements

Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver’s License Knowledge of local community resources; understanding of development and human behavior as it relates to illness/injury and cultural factors Knowledge of state and federal homecare regulatory guidelines (preferred) Experience in home health care (highly preferred); experience with high acuity patients is a plus Experience working with an interdisciplinary team; case management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills; organized What we offer
