Physical Therapist Case Manager: Home Health
Base pay range : $120,000.00/yr - $135,000.00/yr 21st Century Home Health Services (21HHS)
is the largest home health agency in San Francisco with 600+ employees, serving 4,000+ patients across multiple counties. We are expanding into Marin and Sonoma counties. What you’ll do as a Home Health Physical Therapist Case Manager: Provide skilled in-home physical therapy services, evaluate patients, develop care plans, and supervise PTAs per agency and regulatory standards. Collaborate with other clinicians to address patient needs and help individuals live safely and productively at home. Restore function to minimize disability, optimize maximum performance within the patient’s capabilities, and address mobility and independence. Communicate and coordinate with patients’ physicians to ensure high-quality care and report patient progress to the MD.
