Overview

Care Manager/“Designated Care Manager” position at Sunrise Senior Living. Provides high-quality care and services to a consistent group of residents and their families in assisted living/long-term care and reminiscence neighborhoods. Supports residents with hands-on care and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment. Base pay range $20.40/hr - $25.60/hr Essential Duties

RESPONSIBILITIES & QUALIFICATIONS Resident Care Review, read, notate, and initial Daily Log to document resident information. Participate in ISP development and monthly updates. Manage a designated group of residents during the shift and monitor their well-being. Observe, report, and document changes in residents’ conditions (skin, behavior, alertness, weight, diet, activity). Attend daily Cross Over meetings with the lead care manager. Notify supervisor if increased care needs arise; inform supervisor of resident condition changes. Respond to security systems and resident call bells promptly; take appropriate action, including resetting call bells. Greet guests, family members, residents, and team members; answer calls professionally. Assist with continence management and maintain sanitation in resident suites and common areas per procedures. Communicate with families as needed and support residents in maintaining independence and safety, upholding Resident Rights and Sunrise Principles of Service. Assist residents with activities of daily living (ADLs) and encourage participation in ISP-directed activities; engage in life skills and reminiscence activities throughout the day. Maintain a warm, respectful atmosphere and a calm environment; adhere to safety regulations and standard care precautions. Host and participate in daily resident activities. Risk Management and General Safety Ensure community compliance with occupational health and safety requirements; promote risk management programs and safety policies. Practice safety procedures, use PPE, understand SDS, and follow lockout/tagout procedures. Report accidents, unsafe conditions, and hazards immediately. Keep cords, equipment, and hazards out of walkways; ensure compliance with fire codes. Follow proper procedures for exposure to bloodborne pathogens and hazardous materials; report exposures promptly. Safely store oxygen tanks and monitor oxygen levels as required. Housekeeping and Laundry Services Maintain and clean resident rooms; manage laundry as outlined in the ISP; handle dining linens as needed. Complete assigned housekeeping tasks and maintain tidy common areas. Dining Service Serve meals, promote a pleasant dining experience, assist with dining room setup and cleanup, participate in pre-meal meetings. Follow residents’ diets as indicated on ISP; document changes in dining habits; provide room service as needed. Practice safe food handling in accordance with standards. Reminiscence Neighborhood Care Manager Responsibilities EA residents in life skills and enrichment activities; attend afternoon social; implement "Creating Pleasant Days" concept. Integrate residents’ life skills into daily routines and blend multisensory experiences into the day. Monthly letter-writing with residents and families. Training and Team Contribution Act as a team member, contribute to team goals, and model Sunrise Team Member Credo. Support residents and guests through Principles of Service; participate in engagement programs and surveys; attend regular meetings and training sessions; comply with infection control procedures and training requirements. Core Competencies Remain calm in difficult situations; multitask; communicate clearly; strong organizational and time management skills; good judgment and decision-making. Experience and Qualifications High School diploma/GED (as required by regulations); CPR and First Aid as required by regulations; at least 18 years old. Prior experience with seniors preferred; a genuine desire to serve and care for seniors.
