Overview

The Care Manager/Designated Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. The role demonstrates the Mission of Sunrise Senior Living, "to champion quality of life for all seniors," in accordance with applicable laws, Sunrise standards, policies, and procedures to promote the highest degree of quality care and services to residents. Maintain and document resident information by reviewing, reading, notating, and initialing the Daily Log. Participate in ISP development and monthly updates; review designated assignments; supervise a designated group of residents during the shift and check on them throughout the shift. Observe, report, and document changes in residents’ condition (skin, behavior, alertness, weight, diet, participation in activities). Attend daily Cross Over meetings; notify supervisor if a resident requires increased care; inform supervisor of changes in resident condition. Respond promptly to security systems and resident call bells; greet guests, family members, residents, and team members; answer and direct phone calls professionally. Assist with continence management and sanitation; communicate with families as needed; promote independence and safety while upholding Resident Rights and Sunrise Principles of Service. Assist and lead residents with activities of daily living (ADLs) and encourage participation in ISP- indicated activities; engage residents in life skills and reminiscence activities; respond with empathy to each resident’s communication needs. Maintain a warm, respectful, and calm environment; ensure safety regulations and infection control standards are followed. Host and engage in daily activities with residents; manage housekeeping tasks and dining services per ISP specifications. Collaborate with the Reminiscence Neighborhood team on life skills integration and multi-sensory experiences; participate in family communication through monthly letters and activities as indicated. Risk Management and Safety

Ensure community compliance with occupational health and safety regulations; promote risk management programs and adhere to PPE, SDS, and lockout/tagout procedures. Report accidents, unsafe conditions, and hazards immediately; ensure safe storage of oxygen tanks and safe disposal of hazardous materials and body fluids. Training and Team Contribution

Participate as a team member, support engagement programs, attend regular meetings and trainings, and maintain infection control practices as per policies. Maintain required training and certifications as mandated by Sunrise and applicable authorities; perform other duties as assigned. Core Competencies

Ability to stay calm under pressure, manage multiple priorities, and communicate effectively; strong organizational and time-management skills; sound judgment and problem-solving abilities. Experience And Qualifications

High School diploma/GED; CPR and First Aid as required by regulation; must be at least 18 years old. Previous experience working with seniors preferred; desire to serve and care for seniors; ability to act in residents’ best interests. If applicable, drivers must sign the Driver job description and comply with safety requirements. Benefits
