Health and Housing Specialist – Tempe, AZ
Join to apply for the Health and Housing Specialist role at Terros Health. Base pay range: $17.18/hr - $19.21/hr. Plus a $3/hour differential. Full-Time Employed: 40 hours/week. Primary Location: Priest Drive Recovery Health Center, Tempe, AZ. Schedule: Monday – Friday; two shifts available: 7:30am – 4:30pm or 8:30am – 5:30pm. No on-call or weekends. Provide direct face-to-face community-based services to assist clients in obtaining and maintaining housing. Conduct screenings, intake assessments, and develop appropriate treatment plan goals based on presenting problems. Engage clients through outreach and orient potential clients to the H20 program. Provide transportation to clients as appropriate and medically necessary, using a personal vehicle or Terros Health provided vehicle when available. Coordinate care with family, behavioral and general medical providers, and other supports; ensure ongoing communication of clinical information as appropriate. Assist clients with connection to physical health services, including Primary Care services and appointment follow-up. Deliver excellent customer service to clients, internal and external providers, state entities, and other stakeholders as needed. Qualifications
GED/High School Diploma with a minimum of 3 years of behavioral health experience, or an Associate degree with a minimum of 2 years of behavioral health experience. SMI experience preferred; housing and case management experience is a plus. Housing experience is a plus; Training provided. Benefits
