Independent Living Group Leader is responsible for implementing assigned participants” Person Centered Plans (PCP) working with participants and their families to improve socialization, self-help, communication and pre-vocational skills. The Independent Living Group Leader will work with assigned participants at The League and in the community.
Essential Duties and Responsibilities:
Communicate with participants, families, professional staff and community members in a person-centered manner
Develop a positive working relationship with assigned participants and families/caregivers
Provide community-based developmental vocational habilitation services consistent with participants’ PCPs
Develop good working knowledge of participant’s Behavior Plan, if applicable, utilizing strategies and techniques within the plan to assist participant with decreasing target behaviors/increasing communication skills
Implement skill acquisition in areas of , daily living skills, socialization, leisure, basic academics, motor skills, communication, and self-advocacy
Participate in the development, implementation, monitoring and updating of assigned participants’ PCPs
Assist assigned participant with toileting and personal hygiene as needed
Maintain complete and accurate participant dailydata entry in iCare, tracking behavior data, if applicable, and in other required documentation
Other Duties and Responsibilities:
Provide transportation for assigned participants using League vehicles Provide mobility monitoring and/or training, as needed. Assist in the completion of all quality assurance protocols including service satisfaction surveys, program efficiency and outcome measurements. Additional responsibilities as assigned by the Manager, Day Habilitation Demonstrate the 5 Core Values of The League (Accountability, Participant Focus, Quality, Independence and Mutual Respect) Qualification Requirements:
Minimum age of 18. valid driver’s license with no more than 3 points. 2 years of driving experience Must possess basic computer skills including Microsoft Word and Outlook Must possess excellent written and verbal communication skills Educational and/or Experience:
At least one year of experience working with individuals with developmental/behavioral disabilities High School diploma or GED required; AA in related field preferred.
