Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff. Essential Duties and Responsibilities:
Provide primary-care services according to site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities. Quality/Compliance:
Review and sign MARs, sign all orders specific to Primary Care within 24 hours, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review labs within 24 hours of receipt. Manage narcotic prescriptions per policy. Allocate time for administrative paperwork during visits. Provide on-call triage duties as per the protocol policy, adhering to the latest triage schedule, which is determined by a majority vote of providers. Complete documentation and submit billing records according to policies, providing a hard copy within 48 hours for inclusion in the patient’s facility record. Maintain open communication with facility staff and administration, providing verbal and written instructions or feedback. Meet with the facility care coordinator before and after patient visits each day. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical guidelines. Maintain patient confidentiality at all times, complying with HIPAA regulations. Consult with the clinical supervisor before refusing any resident, family, or facility request regarding resident care. Qualifications/Education:
Masters in Nursing from an accredited institution and a valid license to practice medicine in your state, along with current certifications. Or, graduation from an accredited Physician Assistant program, passing the national certification exam, and licensure in your state. Geriatric, family, or psychiatry specialization is preferred.
