Job Overview

The Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include administration of medications, documentation of medication administration, and providing resident care while demonstrating the Sunrise Mission to champion the quality of life for all seniors in accordance with laws, standards, and Sunrise policies. Responsibilities

Medication Administration: Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Documentation: Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents. Updates: Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse. Medication Pass: Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass. Timely Pass: Ensure medications are passed according to times and using the mobile med cart. Documentation of Refusals/Missed Doses: Document and initial as medications are given and ensure appropriate documentation for refusals or missed doses. Confidentiality: Maintain confidentiality of all resident information including medications. Communication: Report resident concerns and unavailable medications to the RCD or Wellness Nurse. Stock and Cleanliness: Restock medication cart after passes; maintain and clean the Medication Room and carts; remove expired medications. Replenishment: Follow refill process and notify if a new prescription is needed. Resident Independence: Help residents maintain independence and promote dignity and safety. Observation: Assess residents for “as needed” medications and document/report appropriately. Controlled Substances: Count narcotics with another MCM or LCM each shift and report discrepancies. Resident Care Communications

Notify RCD of any resident and/or family concerns. Attend and participate in daily Cross Over meetings; participate in development of ISP. Risk Management & General Safety

Ensure compliance with regulations; promote risk management programs and adhere to safety rules. Practice safety procedures (PPE, fire safety, SDS, Lockout Tagout). Report accidents/incidents and unsafe conditions immediately. Keep cords, carts, and equipment clear of exits; comply with infection control techniques and hazardous materials procedures. Store and manage oxygen tanks safely and monitor prescribed levels. Training & Team Success

Participate as a team member and contribute to goals; uphold Team Member Credo and Sunrise Principles of Service. Engage in engagement programs and surveys; attend regular meetings and trainings as directed. Maintain mandatory trainings and compliance with regulatory authorities. Perform other duties as assigned. Core Competencies

High School diploma/GED; may be required per state/provincial regulations Maintain relevant certifications (CPR/First Aid, etc.) as required Must be at least 18 years of age Previous experience working with seniors preferred Desire to serve and care for seniors If applicable, drivers must sign Driver job description and follow safety/regulatory duties Benefits & Additional Information
