The Medication Care Manager is responsible for providing the highest quality of care by administering medications and treatments in a safe, organized manner, in accordance with federal, state/provincial, and local laws, Sunrise policies, and the Mission to champion the quality of life for all seniors. Responsibilities & Qualifications
Essential Duties
Medication Administration Provide the highest quality care by administering medications and treatments in a safe, organized manner. Review, read, document, and initial the electronic health record to learn about residents. Receive medication updates from RCD or Wellness Nurse. Administer, assist with, and observe medications and treatments following the Six Rights of Medication Pass. Ensure medications are passed on time and documented, including refusals or missed doses. Maintain confidentiality of resident information and medications. Report concerns and unavailable medications to the RCD or Wellness Nurse. Restock medication cart after passes; assist in checking medications regardless of packaging system. Assess residents for as-needed medications and document accordingly. Count narcotics with another MCM or LCM each shift and report discrepancies. Maintain and clean the Medication Room and carts; manage inventory and expires appropriately. Follow refill processes and notify if a new prescription is needed. Support resident independence, dignity, and safety in line with Sunrise Principles of Service. Demonstrate empathy and respond to residents’ communication patterns and needs. Follow standard care precautions for cleanliness and hygiene. Resident Care Communications Notify RCD of resident or family concerns. Attend and participate in daily Cross Over meetings and ISP development. Risk Management and General Safety Ensure regulatory compliance in occupational health and safety; promote risk management programs. Practice safety procedures including PPE, fire safety, SDS, and Lockout/Tagout; report accidents or hazards immediately. Keep cords, carts, and equipment clear; comply with infection control and biohazard policies. Handle oxygen tanks safely and monitor prescribed levels. Training and Team Success Collaborate with team, participate in engagement initiatives, and attend required meetings and trainings. Maintain compliance with required trainings to meet Sunrise standards. Core Competencies Make resident-centered decisions; stay calm in difficult situations. Manage multiple priorities with strong written and verbal communication; exercise good judgment and problem solving. Experience And Qualifications High School diploma/GED; certification as required by state/provincial regulations. Maintain CPR and First Aid certifications where applicable. Must be at least 18 years of age; prior experience with seniors preferred. Desire to serve and care for seniors; drivers, if applicable, must understand safety duties.
