Medication Care Manager role at Sunrise of San Rafael. The Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner, in accordance with Sunrise policies and applicable laws. Essential Duties As a part of the Sunrise team, support the Mission, Principles of Service and Core Values. The role includes the essential functions listed below. Medication Administration Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents. Receive medication updates from Resident Care Director (RCD) or Wellness Nurse. Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass. Ensure medications are passed according to times and using the mobile med cart. Document and initial as medications are given and ensure documentation is completed for refusals or missed doses. Maintain confidentiality of all resident information including resident medication information. Report resident concerns and unavailable medications to the RCD or Wellness Nurse. Restock medication cart after all medication passes. Assist in checking medication regardless of packaging system. Assess residents for need of as-needed medications and document/report to supervisor. Count all narcotics with another MCM or LCM each shift and report discrepancies. Maintain and clean the Medication Room, carts, and treatment carts; ensure neatness, availability of medications, and removal of expired medications. Follow the refill process and notify if a new prescription is needed. Help residents maintain independence and promote dignity and safety adhering to the Sunrise Principles of Service. Respond to each resident with empathy, mindful of communication patterns, history, and basic human needs. Practice standard care precautions of cleanliness, hygiene, and health. Resident Care Communications Notify RCD of resident and/or family concerns. Attend and actively participate in daily Cross Over meetings facilitated by the LCM. Participate in the development of Individualized Service Plans (ISP). Risk Management and General Safety Partner with the community team to ensure compliance with regulations pertaining to occupational health and safety; promote risk management programs and policies. Practice safety procedures including PPE, fire safety equipment, SDS, and Lockout Tagout where applicable. Report all accidents/incidents immediately and unsafe conditions/equipment. Keep cords, carts, equipment, and hazards out of exits and in compliance with fire codes. Comply with infection control techniques and bio-hazard disposal policies. Understand and practice proper handling of blood borne pathogens and hazardous materials; report exposures immediately. Ensure safe storage of oxygen tanks and monitor prescribed levels. Training and Team Contribution Participate as a team member and work toward team goals. Demonstrate Sunrise credo in daily interactions and serve residents through Principles of Service. Contribute to engagement programs and processes, including surveys and planning workshops. Attend regular meetings and trainings as directed; maintain compliance with required trainings. Perform other duties as assigned.
