Responsibilities/Accountabilities
Coordinate the day-to-day care of residents in accordance with the service plan, including ensuring sufficient staffing, coordinating staff schedules, and supervising RMAs and direct care staff.
Be knowledgeable of the individualized care plan and provide support to residents according to the care plan; contribute to the care planning process by providing information and observations to the charge nurse or other care planning staff.
Complete any state-required assessment and service plan training.
Coordinate services with hospice and home health to ensure services are provided as ordered by primary care providers and outlined in the service plan; review outside provider communication and participate in wound monitoring/management per Viva Senior Living policies.
Coordinate services with durable medical equipment companies to ensure residents receive needed equipment as directed/ordered by their medical provider.
Coordinate medication management with pharmacies, including managing medication orders, ensuring medication availability, medication destruction, medication cart management, and routine auditing of medication services.
Administer and document medications and treatments per physician orders and accurately record all care provided to residents.
Report care provided and changes in residents’ condition to the responsible party, primary care provider/physician, and Resident Care Director.
Review End of Shift Reports and Med Tech to Med Tech logs and respond accordingly; respond to resident emergencies; document incident reports as needed.
Conduct community audits and training to ensure compliance with state regulations and company policies.
Participate in routine activities of resident care, including daily living and basic nursing procedures.
Respond to inquiries from residents, visitors, and staff promptly and courteously within policy timeframes.
Maintain confidentiality of Protected Health Information and report non-compliance and breaches as required.
Protect residents from abuse and cooperate with investigations; report allegations of abuse or mistreatment as appropriate.
Participate in On-Call rotation and emergency drills; assist with evacuation during emergencies as needed.
Fulfill responsibilities during implementation or activation of the facility’s emergency plan.
Maintain professional interactions with residents, families, and peers; arrive to work on time and perform other duties as required.
Qualifications
Must possess a valid LPN license, in good standing, from an approved TN school of nursing.
Must possess at least a high school diploma or GED.
Must be able to read, write, speak, and understand English.
Must adhere to Viva Senior Living’s attendance policies and employment terms as set forth in the Employee Handbook.
Must maintain the highest standards in caring for residents and adhere to the Resident Bill of Rights.
Physical, Health, and Safety Requirements
Annual health requirements: provide annual verification of a negative TB skin test and annual physical.
Must be able to move intermittently; cope with mental and emotional stress; see and hear or use prosthetics as needed.
Must meet general health requirements including a medical and physical examination.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, or agitated residents.
Must be able to push/pull/move/lift a minimum of 25 pounds to a height of 5 feet, and carry such weight up to 50 feet.
May be required to assist in evacuation of residents during emergencies.
Salary and Benefits Salary:
$72,000
per year
