Outreach Specialist - Better Birth Outcomes Centers For New Horizons is a nonprofit organization that assists children, youth, and families to become self-reliant. With 100+ full-time staff, Centers provides programs in seven locations throughout the Bronzeville and Riverdale communities of Chicago's Southside that reach over 1,000 individuals daily.

Position Description The Outreach Specialist primary responsibility is to assist in finding and informing participating families of resources available to them. The outreach specialist primary objective of outreach activities is to inform potential families and medical, social, or other types of inpatient or outpatient service providers of available services, eligibility criteria, and method of accessing services (for example, the name, address, and phone number of the agency). This is not to preclude the use of nontraditional methods of outreach that may be necessary to identify potential participants in hard-to-reach populations, such as persons with substance use disorder.

Essential Functions

Build and maintain strong relationships in the local community with medical providers, including but not limited to physicians, certified nurse midwives, nurse practitioners, physician assistants, mental health providers, and hospital labor and delivery and emergency room personnel

Build and maintain strong relationships in the local community with social and human services providers, including but not limited to WIC, Early Childhood Home Visiting, Substance Use Prevention and Recovery, Housing Assistance, Welcoming Centers, and Community Based Organizations who provide emergency items, supplies, and services

Establish working relationships between the Local Agency and Medicaid Managed Care Organizations serving individuals within the Local Agency’s service area

Raise awareness to eligible populations on the benefit of the program and promote program enrollment to interested clients

Provide families with information on Family Connects during the prenatal period and to collaborate with the Family Connects program to ensure appropriate handoff

Provide personalized referrals and assist families in connecting with medical, social and community support

Attend required in person trainings and workshops

Serve as a local agency liaison

Adherence to agency policies and procedures

Educate pregnant and parenting clients understand the importance of a medical home

Conduct community outreach and engagement. Outreach can include community campaigns such as production and distribution of brochures/flyers, design and publication of newspaper announcements, billboards, and production and broadcast of public service announcements or paid advertising on radio, television, or social media

Conduct constant evaluation of outreach activities for effectiveness

Assist Program Manager in the completion of outreach plans

Other duties as assigned

Qualifications

Education and Experience Associate degree in a behavioral science, social science, or healthrelated area preferred.

Experience in maternal and child health or public health program coordination preferred

Knowledge, Skills and Abilities

Must have valid Driver’s License, vehicle insurance and daily access to a vehicle.

Background check conducted as a condition of employment.

Salary Compensation is commensurate based on experience, position requirements, and candidate qualifications. The salary $45,000 annually.

Work Environment The typical work environment is outside the office within the Chicagoland Area. The noise level will vary depending on the location. External vehicles, office equipment, machinery, telephones, and conversations produce noise. May require nights, weekends, or holiday work.

While performing the duties of this job, the employee is regularly required to talk and listen. The employee is frequently required to sit, stand, stoop, and walk. The employee must occasionally lift and or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception and the ability to adjust focus. The employee may be required to work in or visit offices in the city of Chicago and suburbs and must be able to transport himself/herself to various locations.

Equipment Used Employees will generally use the telephone, computer, calculator, copier, paper shredder, paper cutter, and filing systems.
