Case Manager with a housing focus through the 1115 Waiver Program , you’ll take on the meaningful role of Housing Advocate for Catholic Charities of Delaware, Otsego, & Schoharie Counties. You’ll be the go-to support for individuals facing housing challenges—helping them find stability, connect with vital resources, and progress toward brighter, more independent futures. Case Manager Minimum Salary: $22.00 / Hour; Case Manager Maximum Salary: $22.00 / Hour Schedule:
Monday – Wednesday, & Friday 8:30AM – 4:00PM; Thursdays 8:30AM - 6:30PM (35 hours per week) Responsibilities
Provide comprehensive housing support to eligible households under the 1115 Waiver, ensuring that all services are documented accurately in the Unite Us platform. Housing services include: Pre-tenancy services: Assisting with rental applications, negotiating lease agreements, setting up new housing units, connecting clients to financial assistance for housing costs, and ensuring the living environment meets their needs.
Stay informed on policies, eligibility requirements, and necessary documentation for programs such as Temporary Assistance, Public Assistance, Section 8 Housing, Rental Assistance, SSI/SSDI, SNAP, HEAP, and other relevant resources. Provide guidance and advocacy to help clients negotiate expanded benefits and advocate for themselves in conferences and hearings. Assist clients in securing stable housing, including: Conducting outreach to landlords and establishing partnerships. Helping clients locate and apply for affordable housing.
Qualifications
Associates Degree in Human Services or related field 1-3 years of experience in mental health, substance use, or human services Benefits
