Description Of Work

This position coordinates the Minimum Data Set (MDS) system according to CMS certification and licensure standards on all residents. Coordinates and serves as a Team Leader for the Treatment Team on the assigned unit. Serves as a teaching role as needed to maintain skills for clinical disciplines on MDS and care planning. Coordinates, initiates, and assesses each resident on admission utilizing the comprehensive MDS system. Coordinates assessments for each resident utilizing the comprehensive MDS system. This includes all admissions, quarterly, annual, and significant change assessments.

Knowledge Skills And Abilities (KSAs)

Basic knowledge of nursing principles and techniques for medical, surgical and mental health practices.

Knowledge and skills in completion of MDS assessments, application of the nursing process, and development of resident treatment plan.

Ability to effectively communicate information to patients, families and staff in a concise and descriptive manner verbally and in written form.

Ability to gather, compile, and perform preliminary analysis on data.

Ability to lead a team and work as a team player with multi-disciplinary health care staff.

Ability to use interview techniques that assist residents in describing his/her symptoms.

Proficient in the use of computers with the ability to navigate and learn new software programs.

Salary Grade: MH16

Hours: Monday - Friday 8am - 5pm with some evening/night/weekend work

This position is designated Mandatory and must report or stay over during adverse weather events.

This position is Exempt and is Exempt from certain provisions of the State Human Resources Act (GS 126).

Shift Premiums For 8-hour Shifts

Day: Saturday & Sunday 10%

Evening: Monday-Friday 20%; Saturday & Sunday 30%

Night: Friday - Sunday 30%; Monday-Thursday 20%

Eligibility for Sign-on Bonus

To qualify for the sign-on bonus, a newly hired permanent full‑time or permanent part‑time employee must work 30 – 40 hours per week to be eligible. Those who are eligible and working less than 40 hours a week will receive a prorated rate.

An employee is not eligible for a sign-on bonus if:

The employee has held a position in North Carolina State government, including the judicial system, or the University of North Carolina system within the last 12 months (the employee shall remain ineligible for 12 months from the separation date)

The employee has previously received a sign‑on bonus from any State agency

DHHS has paid any contract placement or recruitment fees.

Veteran's and National Guard Preference
