The psychiatric hospitals provide state-of-the-art, recovery-oriented, science-based care and are accredited by The Joint Commission. The Social Worker II performs and documents complex social services work including serving as a liaison between the individual, legally authorized representative, and others to assure ongoing care, treatment and support through person-centered practices. This position gathers information to assess the individual\'s support systems and service needs, participates in interdisciplinary team meetings as needed, completes an initial social history, maintains updates, supports assessment of rights and capacity to make decisions, and provides individual and group training to promote rights and self-advocacy. The position designs and provides training to promote rights and coordinates admissions, transfers, transitions, and discharges as needed. This role works under moderate supervision with limited latitude for initiative and independent judgment. Performs other duties as assigned, including participation in disaster response/COOP activation with potential for alternate shift patterns or locations. This position may be eligible for additional pay for work at maximum security locations. Essential Job Functions (EJFs): Attends work regularly and may be scheduled for specific, rotating, or extended shifts; may be on-call as required. Interviews and collaborates with individuals, legally authorized representatives, and involved persons to identify needs and gather background information for psychosocial, economic, and physical needs assessments. Coordinates communication and information exchange among individuals, representatives, involved persons, agencies, and treatment providers; ensures liaison function, notifications, consent access, and interdisciplinary participation. Assists with guardianship issues, annual living options discussions, end-of-life planning, and rights protections per state/federal laws and policies. Oversees rights and capacity assessment coordination and documentation. Assigns staff for admissions, transfers, transitions, and discharges to ensure access to services using person-centered practices; coordinates tours and documents required for admission and transfers, including transportation and information transfer. Designs and conducts individual and group training on rights, voting, citizenship, decision-making, and self-advocacy; facilitates group education to enhance knowledge of rights and privacy. Participates in resident and family meetings and presents facility processes. Performs other duties as assigned, including disaster response/COOP participation; may require alternate shift patterns or locations. Knowledge, Skills and Abilities (KSAs): Knowledge of individuals with intellectual and developmental disabilities, human growth and development, behavioral and mental health, and social work standards for case management, resources, and practice theories. Accountability for assigned property with limited financial responsibilities. Skills in interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, information and referral, problem solving, supervision, consultation and advocacy. Ability to perform duties in accordance with the Texas State Board of Social Work Examiners Code of Conduct, NASW Code of Ethics, and state center policies. Effective written and verbal communication in both individual and group settings. Registrations, Licensure Requirements or Certifications: The Social Worker is required to maintain licensure in accordance with the Texas Board of Social Work Examiners. Initial Screening Criteria: Baccalaureate Social Worker, Master Social Worker, or Clinical Social Worker required. AND One year of experience with persons with mental health issues required, OR completion of State Hospital internship program. Additional Information: Applicants must pass fingerprint background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and Employee Misconduct, LEIE).
