Responsibilities

The Social Worker II performs and documents complex social services work including serving as a liaison between the individual, legally authorized representative, and others to assure ongoing care, treatment and support through person-centered practices.

Gathers information to assess the individual's support systems and service needs, including participation in interdisciplinary team meetings as needed.

Completes an initial social history and maintains appropriate updates; assists with rights assessment and training to promote exercise of rights.

Coordinates admissions, transfers, transitions, and discharges as needed; works under moderate supervision with limited latitude for initiative and independent judgment.

Performs other duties as assigned, including disaster response or Continuity of Operations (COOP) activation; may require alternate shift pattern or location.

Attends regular work and may include specific or rotating shift schedules, overtime, and on-call duties as required.

Interviews individuals and involved parties to determine needs and gather background information; updates as appropriate.

Oversees communication and information exchange among individuals, representatives, and providers to ensure ongoing care and required notifications.

Assists with guardianship issues, living options discussions, end-of-life planning, and rights education in accordance with laws and policies.

Coordinates rights and capacity assessments and reviews documentation; assigns staff for admissions, transfers, transitions, and discharges using person-centered practices.

Designs and conducts individual and group training to promote rights, voting, citizenship, and development of decision-making and self-advocacy skills.

Participates in residence and family meetings and conducts formal presentations describing facility processes.

May serve in an on-call status and participate in COOP activations or disaster response as needed.

Qualifications

Knowledge, Skills And Abilities : Knowledge of individuals with intellectual and developmental disabilities, human growth and development, behavioral and mental health, and social work standards for case management; skills in interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, information and referral, problem solving, supervision, consultation and advocacy.

Ability to communicate effectively in writing and verbally in both individual and group settings.

Registrations, Licensure Requirements Or Certifications : The Social Worker is required to maintain licensure in accordance with Texas Board of Social Work Examiners.

Initial Screening Criteria : Baccalaureate Social Worker, Master Social Worker, or Clinical Social Worker is required. One year of experience working directly with persons with mental health issues is required, OR completion of State Hospital internship program.

Additional Information : Applicants must pass a fingerprint background check, pre-employment drug screen, and registry checks including CANRS and LEIE. Males 18-25 must be registered with Selective Service. All applicants must be at least 18; Waco Center for Youth applicants must be 21. Flexibility in work hours; overtime and extended hours may be required. Immunization policy compliance is required. Telework may be eligible per policy. HHSC uses E-Verify; bring I-9 documentation on first day.
