The Social Worker II performs and documents complex social services work including serving as a liaison between the individual, legally authorized representative, and others to assure ongoing care, treatment and support through the use of person-centered practices. This position gathers information to assess the individual\'s support systems and service needs, including participation in interdisciplinary team meetings, as needed. This position completes an initial social history, as well as maintains appropriate updates. This position may also support the assessment of the individual\'s rights and capacity to make decisions and provides individual and group training to promote the exercise of rights. This position assists with the coordination of admissions, transfers, transitions, and discharges, as needed. The Social Worker II works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Responsibilities Performs other duties as assigned, including disaster response and Continuity of Operations (COOP) activation. May require alternate shift patterns or locations.

This position may be eligible to earn additional pay for work performed on maximum security locations.

Attends work regularly and may work specific or rotating shifts, with possible on-call duties.

Interviews individuals and representatives to determine needs, gather background information, and assess psychosocial, economic, and physical needs.

Oversees communication between individuals, representatives, involved persons, agencies, and providers to ensure ongoing care and required authorizations.

Assists with guardianship issues, living options, end-of-life planning, and rights discussions under applicable laws and center policies.

Coordinates admissions, transfers, transitions, discharges, and related processes to ensure access to services using person-centered practices.

Designs and conducts individual and group training to promote rights, voting, citizenship, and self-advocacy skills.

Participates in residence and family meetings and conducts formal presentations describing facility processes.

Knowledge, Skills and Abilities (KSAs) Knowledge of individuals with intellectual and developmental disabilities, mental health, and social work standards for case management and resource navigation.

Skills in interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, and advocacy.

Ability to communicate effectively in writing and verbally in individual and group settings.

Licensure and Certifications The Social Worker must maintain licensure in accordance with the Texas Board of Social Work Examiners.

Qualifications Baccalaureate Social Worker, Master Social Worker, or Clinical Social Worker required.

One year of experience working directly with persons with mental health issues OR completion of State Hospital internship program.

Additional Information Applicants must pass background checks, pre-employment drug screen, and registry checks as applicable.

All applicants must be at least 18 years of age; Waco Center for Youth applicants must be 21.

Flexibility in work hours; overtime and extended hours may be required.
