Job Description: Medical Social Worker
Medical Social Worker Job Summary: The Medical Social Worker will provide comprehensive social work services in both the hospital and rural health clinic settings. This dual-role position includes hospital-based discharge planning, coordination of post-acute care services, and providing social support to patients in a Rural Health Clinic. The ideal candidate will be flexible, compassionate, and skilled in addressing the holistic needs of individuals facing health challenges.
Responsibilities
Hospital Discharge Planning: Assess psychosocial needs of patients and families during hospitalization. Collaborate with providers, nurses, care managers, and other members of the multidisciplinary team to create and implement effective discharge plans. Coordinate referrals to home health, skilled nursing facilities, rehabilitation centers, hospice, and other community resources. Provide crisis intervention and support to patients and families. Document assessments, plans, and interventions in the EMR in a timely and accurate manner. Educate patients and families about available services, benefits, and community programs. Rural Health Clinic Support: Work closely with primary care providers and interdisciplinary teams to support integrated care models. Support patients facing social determinants of health barriers offering resources and guidance for the patient, family and care teams. Medical Social Worker Department: Care Management; Reports to: Chief Operating Officer (COO) Supervisory Role: No If licensed (LCSW), provide individual, family, and/or group counseling to patients coping with mental health concerns, chronic illness, life transitions, or trauma. Develop and implement individualized treatment plans using evidence-based practices. Qualifications
Education: Master’s degree in social work (MSW) from an accredited program required. Licensure: Must hold a current license in the state (e.g., LMSW, LCSW, or equivalent) Clinical licensure (e.g., LCSW) preferred to provide counseling. Experience: 1–2 years of experience in a healthcare setting preferred. Skills: Strong assessment, problem-solving, and organizational skills. Knowledge of community resources, healthcare regulations, and discharge planning. Ability to work independently and as part of a multidisciplinary team. Excellent written and verbal communication skills. Culturally sensitive and able to work with diverse populations. Professional customer service and communication Proper phone etiquette Computer skills related to EMR, Outlook, MS Word, Excel, TEAMS Ability to work closely with teams in a fast-paced work environment Good organizational and time management skills Medical terminology knowledge Work Environment
