Position Overview
The Case Management Coordinator plays a central role in ensuring that immigrant and refugee individuals and families receive coordinated, effective, and culturally responsive support across AHI’s programs — including PRIME, Wellness, Employment, and Youth Empowerment. This position connects the dots between programs and partners, ensuring that participants experience seamless care, consistent follow-up, and sustained progress toward stability and self-sufficiency. The Coordinator will balance direct case management with systems-level coordination, helping to identify gaps, strengthen internal referrals, and advocate for participants navigating complex service systems. The Case Management Coordinator will report directly to the Program Director and work closely with the cross-program coordinator, and other program staff. This position does not currently supervise other staff but will provide mentorship, guidance, and support to case managers, interns, and volunteers as needed. Key Responsibilities
Coordinate case management activities across multiple AHI programs to prevent duplication and ensure holistic client support. Track participant progress across service areas, aligning follow-up and documentation. Strengthen referral pathways between internal teams and external partners (housing, healthcare, employment, education, etc.). Develop centralized database for case management resources. Client Case Management & Problem Solving
Conduct comprehensive intake assessments to identify individual and family needs. Develop personalized navigation plans, assist with applications and eligibility documentation, and ensure services are accessed. Follow up consistently to confirm services are received and goals are being met. Cultural Responsiveness & Trust-Building
Provide trauma-informed, strengths-based support that reflects empathy and understanding of displacement, resettlement, and adjustment experiences. Communicate in ways that honor cultural norms and language preferences, ensuring comprehension and comfort. Build long-term, trust-based relationships that foster self-sufficiency and confidence. Identify and help resolve barriers such as transportation, documentation, and system access challenges. Advocate with agencies and service providers to ensure clients are treated equitably and receive appropriate accommodations. Collaborate across systems to remove structural barriers that disproportionately affect immigrant and refugee families. Partnership & Resource Development
Maintain up-to-date knowledge of local resources and referral pathways. Strengthen partnerships with community agencies and public systems. Identify emerging needs and advocate for systemic improvements. Data, Compliance & Evaluation
Maintain accurate and timely documentation of all case management activities in AHI’s databases (e.g., SharePoint, CaseWorthy). Monitor caseloads and ensure client records, consent forms, and wellness surveys are complete and up to date. Prepare monthly, quarterly, and annual reports as required by funders and AHI leadership. Ensure compliance with DSHS, ORIA, PHPDA, and other contract requirements, as well as AHI’s internal data security and confidentiality policies. Track and evaluate participant progress toward program goals (employment, wellness, stability, self-sufficiency). Gather client feedback and success stories to inform program development. Program Visibility & Community Engagement
Help increase awareness of AHI’s programs by sharing stories, updates, and highlights through social media and community outreach. Create and support dissemination of short videos and community-facing content that highlight AHI’s impact and connect clients to available resources. Collaborate with Program Teams and the Development Team to share client success stories and promote culturally responsive initiatives. Administrative & Other Duties
Handle sensitive information with discretion, following AHI’s client confidentiality and data protection standards. Flexibility to occasionally work evenings or weekends to support workshops, outreach, or community events. Qualifications
Required Associate’s or Bachelor’s degree in Social Services, Human Services, Public Health, or a related field. At least 1–2 years of case management or client navigation experience in a nonprofit, government, or community-based setting. Demonstrated ability to support immigrant and refugee individuals and families in accessing housing, healthcare, benefits, and other essential services. Strong organizational and documentation skills, including experience with data systems, case notes, and tracking client outcomes. Proven ability to advocate for clients, coordinate with service providers, and resolve complex barriers. Culturally responsive and trauma-informed approach grounded in empathy, respect, and accountability. Excellent English communication skills (speaking, reading, and writing). Knowledge of Afghan and/or Middle Eastern cultural values, with lived experience or deep community connection preferred. Access to reliable transportation and flexibility to work in field-based settings across South King County. Preferred Bilingual/Multilingual (Dari, Farsi, Pashto, Arabic, or other languages common among AHI participants). Experience with multi-program or cross-contract coordination (e.g., ORIA, PRIME, Wellness). Knowledge of local health, housing, and human services systems in King County. Familiarity with data platforms (CaseWorthy, Salesforce, Apricot, etc.) and outcome tracking. Background in advocacy, resource navigation, or systems improvement for immigrant and refugee communities. This is a full-time in person, 40-hour per week position based at AHI’s office in Covington, WA. Standard work hours are Monday through Friday, 9:00 AM to 5:00 PM, with flexibility required for some evenings and weekends. Hourly Rate: $23.33–$27.50 Medical, Dental, and Vision Insurance (AHI covers 99% of premiums and deductibles). Paid Time Off, including vacation, holidays, and sick leave.
