Position Overview The Community Health Navigator (CHN) leads AHI’s health navigation and community outreach portfolio. The role supports individuals and families as they access care, understand options, and connect with resources that promote overall well-being. Focus areas include family and maternal health, healthy homes and environments, mental and emotional wellness, and system navigation. The CHN also serves as a bridge between the community and healthcare systems; stewarding partnerships with healthcare providers, clinics, and agencies to strengthen coordination of care, improve cultural responsiveness, and ensure equitable access to services. This position embodies AHI’s commitment to ensuring that every person can navigate care confidently and that providers have the tools and understanding to meet communities where they are. Key Responsibilities

1. Client Navigation and Case Support Provide individualized support for clients accessing primary, maternal, and preventive health care, behavioral health, and community resources such as housing, food, transportation, and financial assistance. Assist with scheduling, coverage navigation, language support, and follow-up to ensure clients understand and complete their care plans. Maintain accurate case notes, consent forms, and referral documentation in AHI’s systems. 2. Community Education and Engagement Develop and facilitate educational workshops and outreach sessions on topics that promote community health and wellness. Collaborate with residents to co-create culturally and linguistically responsive materials that reflect lived experiences. Participate in community events and spaces to build awareness and trust. 3. Healthcare Provider and Partner Engagement Serve as a liaison between AHI, healthcare providers, and partner organizations to strengthen coordination of care and referrals. Build and nurture relationships with clinics, hospitals, and agencies to promote culturally responsive service delivery. Support training and dialogue with providers to increase understanding of immigrant and refugee health experiences and community needs. Represent AHI in coalitions, learning networks, and advisory groups that align with health equity priorities. 4. Data, Learning, and Quality Improvement Track outreach activities, service numbers, and outcomes using AHI’s internal systems. Use data and feedback to identify trends, gaps, and opportunities for improvement. Contribute to program reports and learning summaries that capture both metrics and stories of impact. 5. Community Voice and Systems Change Gather and elevate community feedback to inform program development and advocacy. Facilitate small group discussions, listening sessions, and leadership opportunities that strengthen confidence and peer connection. Contribute to AHI’s ongoing efforts to shape policies and partnerships that advance health equity. 6. Program Integration and Growth Collaborate across AHI teams to align outreach and communications strategies. Contribute to the design and piloting of new navigation approaches that address emerging community needs. Support the development of cross-sector collaborations that connect health, housing, education, and social services. Additional Duties Participate in staff meetings, trainings, and professional development. Represent AHI at community events and coalition activities. Perform other duties as needed to advance maternal health equity. Qualifications

Required Associate’s or Bachelor’s degree in Public Health or Social Services. At least 1–2 years of program or project coordination experience, ideally in health, maternal health, or social services. Strong skills in community outreach, facilitation, and cross-cultural communication. Experience with data management systems and reporting for health or social programs. Problem-solving skills to address barriers clients face in accessing healthcare. Excellent English communication skills (speaking, reading, writing). Strong interpersonal and organizational skills. Knowledge of Afghan cultural values and lived community experience. Bilingual/Trilingual (Dari, Farsi, Pashto, and English). Field-based community experience. Own means of transportation and flexible availability. Preferred Experience working with immigrant/refugee or diverse communities. Knowledge of maternal health disparities and culturally responsive health promotion. This is a full-time in person, 40-hour per week position based at AHI’s office in Covington, WA. Standard work hours are Monday through Friday, 9:00 AM to 5:00 PM, with flexibility required for some evenings and weekends. Hourly Rate:

$23.33–$27.50 Medical, Dental, and Vision Insurance (AHI covers 99% of premiums and deductibles). Paid Time Off, including vacation, holidays, and sick leave.
