Shift pattern: Monday‑Thursday, 9am‑5pm; Friday, 9am‑4:30pm.
Key Responsibilities
Manage a caseload, providing comprehensive and holistic assessment, planning, implementation and evaluation of health and social care needs for older adults with complex mental health and social care issues.
Work in partnership with patients, carers and other professionals/agencies, demonstrating compassionate caring behaviours to fulfill the key functions of the OACMHT.
Work in a variety of home or near‑to‑home community environments, promoting partnership working in line with legislative frameworks, agreed standards and clinical policies.
Undertake duties without direct supervision, supporting and supervising the work of more junior nursing staff and nursing students, including caseload line‑management supervision and clinical supervision.
Provide line‑management supervision, clinical supervision, mentorship and preceptorship responsibilities, contributing to the learning and development of the staff team.
