The Case Manager IV performs advanced (senior‑level) case‑management work, coordinating patient, family, legally authorized representatives, and Local Mental Health Authority (LMHA) throughout assessment, intervention, planning, and discharge.
Essential Job Functions
Collects social assessment information by interviewing patients, family, and LMHA to determine support systems and discharge needs.
Creates individualized recovery plans with measurable goals, ensuring active involvement through one‑on‑one meetings and family contact.
Advocates for individual rights, providing assistance regarding voting, privacy, and other rights in compliance with state and federal law.
Educates family on hospital rules and mental illness, coordinates family visits and supports continuity of care.
Conducts group and individual interventions focused on discharge planning, social skills, community readiness, and mental health skill development.
Collaborates with the recovery team to develop and document discharge plans, ensuring safe transport and provider hand‑off.
Assists and trains other Case Managers I, provides in‑service training quarterly, and participates in quality‑assurance activities.
Performs additional duties as assigned, including disaster response, continuity of operations, and support for maximum security locations.
Knowledge, Skills, and Abilities (KSAs)
Knowledge of community resources and procurement of information on those resources.
Knowledge of case‑management principles, objectives, standards, and methods.
Knowledge of program policies and procedures, including civil and forensic commitments.
Knowledge of competency restoration requirements and the NGRI process.
Computer skills and applicable software proficiency.
Effective oral and written communication skills.
Ability to collect, analyze, and use program data for quality and outcome monitoring.
Ability to assess patient needs, coordinate services, and monitor program effectiveness.
Ability to plan, assign, and supervise the work of others.
Licensure and Certifications None required.
Initial Screening Criteria
Bachelor’s degree with a minimum of six months case‑management or social services experience, OR
Associate degree with a minimum of twenty‑four months experience, OR
No degree with a minimum of forty‑eight months experience.
AND four (4) years of experience in case‑management or social services work.
Additional Information
Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks (CANRS, Nurse Aid, Medication Aide, and Employee Misconduct). Males 18‑25 must register with Selective Service.
All applicants must be at least 18 years of age; the Waco Center for Youth requires applicants to be at least 21 years of age.
Flexibility in work hours may be required, including overtime or extended hours.
Compliance with HHSC immunization policy and state hospital operating procedures related to immunizations is required.
ADA Accommodations HHSC and DSHS agencies will provide reasonable accommodations during the hiring and selection process for qualified individuals with a disability.
Pre‑Employment Checks and Work Eligibility Applicants may be required to pass background and other due diligence checks. HHSC uses E‑Verify. Must bring I‑9 documentation on the first day of work.
