Empath Health is seeking a compassionate
Full‑Time Physical Therapist
to join our interdisciplinary PACE team in
Pinellas Park, FL . In this role, you’ll help older adults age in place with dignity, delivering personalized therapy across homes, facilities, and our Day Center. This is more than a job—it’s a mission in the Program of All‑Inclusive Care for the Elderly (PACE). What You’ll Do
Deliver skilled PT interventions across home, Day Center, and facility settings. Collaborate with the interdisciplinary care team to create personalized therapy plans. Track participant progress and manage documentation. Coordinate use of durable medical equipment (DME). Support restorative, maintenance, and rehab goals in a proactive care model. Position Requirements
Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred). Active (or eligible) Florida PT license. Minimum of 1 year of experience working with geriatric populations. Current CPR certification. Strong communication, time management, and documentation skills. Benefits
