Primary Responsibilities The LPN is responsible for providing daily care, meeting the health needs, ensuring safety and comfort of residents, conducting professional assessments, documenting resident health, and assisting in the management of assigned nursing unit staff.
Essential Functions
Supervise, perform, and document written/verbal care assignments and routine skills and procedures to ensure quality of care to residents
Participate in development and implementation of resident care plans, maintaining up‑to‑date nursing care plans, documenting progress, and making recommendations for revisions
Make daily and periodic visits to observe for symptoms, medication reactions, or other changes
Obtain and document vital signs, intake and output, weights, diet supplements and replacements, cultures, etc.
Assist with orientation and ongoing training of staff to maintain required skill levels for all staff
Report and document resident and/or visitor incidents, accidents, complaints or allegations per facility policy using designated occurrence reporting system
Qualifications
Graduate of an accredited nursing school with a valid LPN license in good standing per State and Federal requirements
Thorough understanding of safe and effective nursing practices in Indiana
Basic computer skills, including Excel, Word, etc.
Personal integrity and ability to deal tactfully with residents, families, visitors, government agencies and personnel
Seniority level: Entry level
