The Division of Correctional Health Services (CHS)

of New York City Health + Hospitals (H+H), responsible for medical, mental health care, and discharge planning in the jail system. Provides comprehensive mental health services, including specialty housing, 24-hour psychiatric service, creative arts, DBT training, clinics, substance use treatment, re-entry services, and partnerships with training institutions. The Patient Care Associate assists licensed staff by performing clinical and clerical duties, including venipuncture, finger-sticks, HIV testing and counseling, emergency response, telehealth facilitation, and patient care activities like ADLs. They observe patient conditions and report changes to nursing staff. Responsibilities

Facilitate telehealth encounters Administer total care (bed bath, partial or complete) Assist with personal hygiene Transfer patients, including physical and mechanical lifting Position patients and perform range of motion exercises Assist with ambulation and ADLs Prepare patients for meals and assist with feeding Assist with admissions and transfers Make beds and tidy patient areas Record vital signs and perform EKGs Apply hot/cold treatments under supervision Maintain intake/output records Notify staff of patient status changes Perform urine testing Chaperone staff in clinical areas Maintain competency in nursing skills Assist with nursing follow-ups and transmit lab info Maintain a safe environment Document encounters in electronic records Complete encounter forms each shift Activities

Perform HIV testing and counseling Enter data into databases Fax testing info to nursing Perform quality controls and EKGs Assist with admissions and venipuncture Unit Maintenance

Clean equipment Replenish supplies Maintain cleanliness of treatment areas Ensure security of keys and equipment Human Relations Skills

Assist colleagues Be responsible and cooperative Maintain professionalism and appearance Accept feedback and communicate effectively Respect individuals and recognize limitations Working Conditions

Includes days, nights, weekends, and holidays. Long standing periods, patient handling, exposure to hazards, and treating uncooperative patients. Physical Demands

Lift up to 50 pounds, possibly 100 Stand for long periods Fine motor skills and visual acuity Qualifications

High School Diploma or GED EKG and Phlebotomy Certificates BCLS Certification and computer skills HIV counseling training preferred PCA/PCT/CNA and Infection Control Certificates Benefits
