Job Description
Provide in-home nursing care to patients following established care plans.
Perform accurate and timely charting and documentation.
Conduct patient assessments and monitor changes in condition.
Coordinate care and communicate updates with clinical managers and healthcare providers.
Educate patients and families on treatment plans and home care instructions.
Ensure compliance with safety standards and home health protocols.
Travel to patient homes as assigned; mileage reimbursed.
Position Requirements
Strong home health knowledge and ability to work independently.
Excellent communication and patient-interaction skills.
Ability to manage time effectively and handle multiple visits per day.
Clean driver’s abstract and reliable transportation.
Ability to follow established protocols and maintain professionalism.
