Director of Case Management - Hospital Discharge Planning Expert
We are seeking an experienced and dynamic
Director of Case Management
to lead our hospital's care coordination and discharge planning initiatives. This strategic leadership role requires a proven healthcare professional with extensive hospital discharge planning experience who can optimize clinical resources, ensure regulatory compliance, and deliver exceptional patient care outcomes through effective case management operations.
Key Responsibilities
Direct and manage all hospital case management department operations, staff, and daily activities
Lead hospital discharge planning processes and ensure seamless care transitions for patients
Coordinate integration of patient care services across interdisciplinary teams and external healthcare agencies
Ensure department compliance with organizational policies, CMS regulations, and external regulatory agency standards
Optimize clinical resource utilization while maintaining quality patient care and continuity of care
Develop and implement case management initiatives and strategic improvements
Build and maintain collaborative relationships with hospital departments and community stakeholders
Monitor and achieve established departmental performance metrics and outcomes
Provide subject matter expertise in case management best practices and regulatory requirements
Lead change management initiatives and foster team development within the department
Present reports and recommendations to executive leadership and stakeholders
Required Qualifications
Bachelor's degree in Nursing, Healthcare Administration, Social Work, or related field (required)
Active Registered Nurse (RN) license, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) credential (required)
Minimum 3 years of recent experience in acute care hospital case management (required)
Hospital discharge planning experience is mandatory
Minimum 2 years of case management leadership or supervisory experience
Demonstrated executive presence with excellent communication and presentation skills
Proven track record of achieving goals and delivering measurable results
Strong leadership, team building, and trust building capabilities
Self-motivated professional capable of managing multiple priorities with minimal supervision
Ability to demonstrate tact and diplomacy in conflict resolution situations
Adaptable to change and skilled at coordinating efforts across diverse stakeholders
Preferred Qualifications
Master's degree in Nursing (MSN), Healthcare Administration (MHA), or Business Administration (MBA)
Case Management certification (CCM, ACM-RN, or similar)
Experience with utilization review and quality improvement initiatives
