Overview We are seeking a dedicated and compassionate Wound Care Provider to join our team. The ideal candidate will have a diverse skill set and experience in various healthcare settings, providing exceptional care to patients of all ages. This role requires a commitment to improving the quality of life for those in our care, whether in a hospital, assisted living facility, or home care environment. Duties Provide direct patient care, including assistance with daily living activities and personal hygiene. Administer medications and treatments as prescribed by healthcare professionals. Conduct patient assessments and monitor vital signs to ensure optimal health. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Assist with diagnostic evaluations and procedures such as IV infusion, catheterization, and specimen collection. Support patients with chronic conditions through education on disease management and health coaching. Facilitate communication between patients, families, and healthcare providers regarding care needs and progress. Skills Candidates should possess a combination of the following skills: Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks. Experience in critical care settings, including ICU or emergency medicine. Knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding, and documentation standards. Familiarity with procedures related to gastroenterology, dialysis, pain management, and behavioral health. Competence in performing physical examinations, venipuncture, phlebotomy, and administering injections. Experience working with diverse populations including pediatrics, geriatrics, and individuals with developmental disabilities. Understanding of infection control practices and aseptic techniques. Ability to provide compassionate end-of-life care in hospice and palliative settings. Strong communication skills for effective patient interaction and family education.
