Home Health LPN Compensation •
$37.30 per hour • Mileage reimbursement at
$0.46 per mile • Weekly time & expense sheet submission • Benefits available to full-time employees after 90 days • 401(k) with company match available after eligibility requirements are met
Home Health LPN Highlights • Contract-to-hire opportunity • Onsite role serving patients in their homes • Schedule:
Monday–Friday, 8:00 AM – 5:00 PM • Weekend on-call rotation required • Coverage area includes
Harrison, Crawford, partial Floyd, and Washington counties
Home Health LPN Responsibilities • Provide skilled nursing care in accordance with each patient’s plan of care • Monitor vital signs and report abnormal findings • Administer medications, feedings, oxygen, ostomy care, and other treatments as ordered • Perform wound care, drain care, and G-tube care as needed • Collaborate with PT, OT, and Speech Therapy to complete established care routines • Maintain strict infection control practices and report potential communicable diseases • Accurately document all nursing care, patient progress, and outcomes • Order and manage medical supplies, medications, and feedings • Ensure patient needs are met per physician instructions • Monitor, assess, and document incidents, completing reports as required • Inspect and maintain medical equipment and report issues for repair
Home Health LPN Requirements • Active
LPN license
(compact license accepted) • Ability to work full-time, 40 hours per week • Reliable transportation for home visits • Strong documentation and communication skills
Preferred Qualifications • Home health experience • Wound care, drain care, and G-tube experience • Experience with
Homecare Homebase (HCHB) • Patient education experience If you want the
