Field RN Case Manager Compensation •

$56.14 per hour • Mileage reimbursement at

$0.46 per mile • Benefits available to full-time employees after 90 days of employment • 401(k) with company match available after eligibility requirements are met

Monday – Friday, 8:00 AM – 5:00 PM • Weekend on-call rotation required • Territory coverage includes

Harrison, Crawford, parts of Floyd, and Washington counties

Field RN Case Manager Responsibilities • Perform comprehensive assessments of high-risk patients • Develop and implement individualized care plans in collaboration with providers • Coordinate care transitions and appropriate levels of care • Work closely with physicians, social workers, discharge planners, and care teams • Identify and arrange non-medical support services (housing, transportation, etc.) • Monitor clinical, functional, and cost outcomes • Promote health education, illness prevention, and patient engagement • Maintain accurate clinical documentation and reports • Act as a patient advocate while ensuring privacy and confidentiality

Field RN Case Manager Requirements • Active, unrestricted

Registered Nurse (RN) license • Compact license accepted • Associate’s or Bachelor’s Degree in Nursing • Strong clinical assessment and decision-making skills • Ability to manage complex medical information • Case Management certification preferred

Field RN Case Manager Preferred Qualifications • Home health or field-based case management experience • Wound care, IV therapy, drain care, and G-tube experience • Experience with patient education • HCHB experience preferred If you think this
