Primary Care Physician (PCP) Role Overview The PCP demonstrates accountability for outcomes, provides strong clinical care to a panel of up to 450 patients, and is a health coach rather than a consultant. The PCP follows value‑based metrics and works toward partnership status. They lead care teams, collaborate with specialists, and ensure compliance with regulations.
Essential Duties and Responsibilities
Provide independent care for patients with acute and chronic illnesses among older adults.
Take full accountability for patient care and outcomes; seek specialist consultation when needed.
Engage with hospitalists for patients in inpatient settings, regardless of hospital affiliation.
Assess, diagnose, treat, manage, educate, and coordinate care, including documentation for complex chronic health needs.
Lead care team consisting of medical assistants, facilitators, and coordinators for office‑based patients.
Collaborate with transition‑care teams (hospital, SNF, LTC, homebound) for patients unable to come to the office.
Actively assist in recruiting patients for the center and recruiting additional providers.
Supervise and collaborate with Advanced Practice Practitioners, remaining accountable for their actions.
Perform other duties as assigned.
Knowledge, Skills & Abilities
Availability and accessibility to patients through walk‑ins, phone calls, and timely responses.
Service orientation and high‑quality patient care.
Evidence‑based medicine with emphasis on lifestyle and nutrition.
Leadership and continuous improvement of practice operations.
Influence and compassionate communication with patients and teams.
Self‑care for physician wellness.
Proficiency in Microsoft Office Suite.
Ability to travel locally and occasionally nationally for training or recruiting.
Fluency in English; proficiency in other languages may be required at some centers.
Education and Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
Active State Medical License for the state(s) of employment.
Board certification or eligibility in the required specialty (Internal Medicine, Family Medicine, Geriatrics).
Current DEA number for Schedule II‑V controlled substances.
Basic Life Support (BLS) certification within first 90 days of employment.
Compensation Pay Range: $214,700 – $306,714 salary.
