Chronic Disease Prevention Program Director

Expected Outcome of Role As the Chronic Disease Prevention Program Director, you are a critical component to assisting individuals, businesses and organizations with the behavior change model in a healthier lifestyle through engagement, encouragement, and meaningful connections. The ability to lead a team of peers, personally connect members to the Y and each other, be highly organized and possess quality character for collaborating with co-workers, members, associates, volunteers, donors and the general public will result in the excellence in service desired.
Responsibilities
Implement innovative tactics to engage new and returning members and create sales opportunities outside of the Y facility.
Lead a high-performing and engaging chronic disease program across the association, working closely with the membership team to develop and manage 4,500+ membership units.
Maintain a portion of the community phase of the branch’s Annual Giving Campaign.
Supervise, coach and mentor a high-performing wellness department including Healthy Living Coaches and Group Fitness Instructors to achieve metric targets and produce meaningful qualitative results.
Develop and manage Chronic Disease Program operating budgets, collaborating with Membership to meet or exceed budget targets.
Provide backup coverage to fitness classes and programs in the absence of instructors to minimize member disruptions.
Analyze program data to ensure quality and performance metrics are met and continuously drive program innovation.
Qualifications
Bachelor’s Degree in Health Sciences, Public Health, or related field and commensurate work experience in fitness/wellness, sales, public relations, and marketing.
Previous management experience in recruitment, training and supervision of staff and volunteer groups.
Willingness and ability to work a flexible schedule, including evenings and weekends.
Excellent problem solving, organizational and communication skills.
Strong leadership and customer service skills.
A positive attitude and the ability to work creatively and independently or as part of a team.
Sound judgment and decision making, and interpersonal skills to relate to people of all socio-economic levels and diverse backgrounds.
Ability to travel locally for meetings.
Essential Functions
Uphold the Mission of the YMCA, demonstrating behaviors that reflect a determined, nurturing, genuine, hopeful, and welcoming nature.
Develop and manage the Chronic Disease Program’s operating budget and collaborate with Membership to meet or exceed budget targets.
Serve as a liaison for the Annual Giving Campaign, developing ongoing volunteer and donor cultivation and stewardship practices, and actively recruiting volunteers.
Develop and maintain relationships with public health, physicians, and payer communities to enhance awareness and referrals to the chronic disease programs.
Serve as a liaison, ambassador, and advocate for the programs within public health forums.
Collaborate with the Director of Marketing and Communications for coordinating member communications (e‑blasts, website, social media, etc.).
Lead the Member Survey Action Team and achieve high member satisfaction rates.
Hire, supervise, train, evaluate, and support all CDP staff.
Recruit, screen, and enroll participants into the program; organize and maintain a master schedule of classes across the association.
Analyze program data to ensure they meet quality and performance metrics and drive continuous improvement.
