Job Description
The Social Worker - Care Manager delivers extensive clinical social work services, which encompass psychosocial evaluations, treatment planning, crisis management, and resource referrals, primarily within acute medical or mental health care environments. This position requires collaboration with patients, their families, and multidisciplinary health teams to enhance patient outcomes and optimize the use of healthcare resources. This role operates during Night Shifts.
Eligible external hires may receive a $5,000 hiring bonus, contingent upon meeting the required qualifications and conditions for payment.

Care Manager MSWThe Clinical Social Worker collaborates with patients, families, and the healthcare team to address and advocate for the social and emotional needs of patients. Responsibilities include providing a comprehensive range of social work services such as psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis management, and resource referrals. The Clinical Social Worker works closely with patients and their support systems, as well as interdisciplinary teams, both within and outside the organization, to enhance patient care through effective resource utilization and monitoring. This role also involves taking a leadership position to help achieve patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and the organization’s mission and vision.
Required Qualifications:
- Master’s Degree in Social Work from an accredited institution.
- Upon hiring: Washington Clinical Independent Social Worker Associate License.
- Minimum of 1 year of experience in acute medical or mental healthcare settings, either inpatient or outpatient.
Salary Range: Minimum: $45.66, Maximum: $69.47
Preferred Qualifications:
- ACM/CCM certification.
The Clinical Social Worker in this role also partners with patients, families, and the healthcare team to address and advocate for patients' social and emotional needs. This role also involves leadership in achieving patient goals and desired clinical, financial, and resource outcomes, adhering to the NASW Code of Ethics and the organization’s mission and vision.
Required Qualifications:

The salary range provided reflects the base pay for this position; additional compensation may be available, including shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonuses.
