Description As the QIDP you will be responsible for coordinating the delivery of each Person's habilitation plan; negotiating and facilitating the array of services and supports needed to address goals as identified through evaluations, assessments, and the summarized plan; monitoring the implementation of the plan; and providing limited supervision to those responsible for program implementation. This position requires frequent contact with the person, their families, if applicable, or LAR, service providers within and outside the agency, State auditors/surveyors, and the community at large.
With limited supervision, you will provide on-going services for up to 24 persons with developmental disabilities by coordinating and facilitating plans and services. This position requires initiative, self-direction, and independent judgment based upon regulations/standards/policies, flexible and sometimes unusual and longer work hours to meet the needs of the Persons (including crisis intervention activities), and the timely completion of required documentation.
Minimum Requirements:
Bachelor's Degree
Advanced degree from an accredited college or university
One (1) year working experience with people with an intellectual disability or related condition
Valid driver’s license with less than 10 pts against driving record verifiable through TX DOT
No supervisory experience required
