Veteran Case Manager

Responsibilities

Provide direct care and ongoing services to Veterans enrolled in MHMR Tarrant supportive services.

Assist Veterans with housing, treatment, medical, social, vocational, financial, educational services to achieve self-sufficiency.

Coordinate continuity of care across the spectrum of mental health services for U.S. Armed Forces service members/veterans/veteran families.

Qualifications

Bachelor’s degree in social, behavioral, health and human services or nursing.

Two years of experience in veteran services or related field.

Valid Texas Driver’s License.

Veteran preferred.
