EMPOWER Service Coordinator
to support individuals enrolled in Empower programs—including Recreation & Wellness and Transition Services—across assigned locations. Your Role in Our Mission As an
EMPOWER Service Coordinator , you will ensure that individuals and families receive exceptional, person-centered services that align with state, federal, and agency standards. You’ll oversee enrollment processes, monitor service quality, collaborate with community and MCO partners, and support staff delivering direct care. This is a Full-Time position, Monday – Friday 9:00am-5:30pm.
In This Role, You Will Enrollment, Planning & Service Coordination
Complete required enrollment documentation for EHR and Provider Direct and submit for billing.
Ensure all necessary consents, authorizations, and approvals are in place before services begin.
Oversee the referral process, including development of person-centered plans, short-term goals, consents, and NCSNAP (when required).
Report quarterly on progress toward goals to supervisors, staff, and families.
Collaborate with MCOs, Empower management, internal service lines, and external partners to ensure supports reflect individual needs and preferences.
Quality Assurance & Compliance
Ensure quality services that promote personal outcomes while protecting participant rights, health, and safety.
Review and approve documentation for direct care services in compliance with Innovations Waiver, Medicaid, and NC funding requirements.
Conduct and submit Clinical Monitoring as required by funding sources.
Maintain accurate participant information in the EHR.
Attend Care Coordination meetings as needed.
Follow all ESPH policies, procedures, and valued behaviors of Accountability, Commitment, and Caring.
Support, Collaboration & Professional Practice
Collaborate with internal and external providers to ensure continuity of services.
Support other service areas—including Recreation Paraprofessionals and Job Coaches—when needed.
Protect and advocate for the rights, dignity, and safety of individuals served.
Maintain regular attendance and appropriate use of PTO.
Complete all required training (NC-SNAP, Relias, etc.) on schedule.
Participate in ongoing professional development.
Perform other related duties as assigned.

$46,800 annually (based on experience)

What We’re Looking For
Minimum 2 years of experience supporting individuals with I/DD
Must meet Qualified Professional (QP) standards
Additional Requirements
Valid driver’s license, clean MVR, and personal auto insurance
Satisfactory criminal background check
Proficiency in Microsoft Word, Excel, Teams, Zoom; EHR experience preferred
Strong attention to detail, organization, and communication
Ability to work independently, multitask, and meet deadlines
Ability to travel to homes, community settings, and program sites
Commitment to confidentiality and compliance standards
