Senior Social Worker

This full-time, permanent Social Work position is located at the Boise VA Medical Center (BVAMC) within the Geriatric and Extended Care (GEC) Service Line of Nursing Service. The incumbent has broad clinical responsibilities as a key member of the Home-Based Primary Care (HBPC) treatment team at the BVAMC.
Pay Range Base pay range: $88,621.00/yr - $115,213.00/yr
Qualifications
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens (VA Policy).
English Language Proficiency – Must be proficient in written and spoken English.
Education – Master’s degree in social work from a school fully accredited by CSWE.
Licensure – Must be licensed or certified by a state to practice social work at the master’s level.
Experience – Minimum two years post advanced practice licensure and at least one year at GS‑11 equivalence.
Advanced practice skills – Demonstrated expertise, advanced knowledge, certification in specialty area.
Knowledge, Skills, and Abilities – Specialized interventions, psychosocial diagnosis, outcome evaluation, consultation, policy development.
Preferred – Experience in Medical Social Work, Primary Care, and Case Management.
Duties
Provide comprehensive psychosocial assessments and integrate into care plans.
Develop treatment plans with veteran, family, and interdisciplinary team.
Conduct home visits and ongoing assessments, update plans quarterly.
Address needs of homebound, frail, disabling/chronic, and dying veterans.
Assist Program and Medical Directors in consult reviews, care planning, systems changes.
Serve as community liaison; educate resources and providers.
Lead staff development, in‑service training, and continuing education.
Participate in committees, work groups, task forces at facility, VISN, national levels.
Coordinate community-based services and referrals.
Provide crisis intervention and case management services.
Maintain therapeutic relationships with veterans and families.
Apply advanced practice skills, professional judgment to assessments and crisis management.
Work schedule: 8:00 am‑4:30 pm Monday‑Friday; Telework/Virtual unavailable.
