Responsibilities Patient Intake & Documentation
Perform and document patient intake in accordance with established protocols.
Collect accurate patient histories, vital signs, vision and hearing screenings, and other assessments or questionnaires as required.
Ensure all information is accurately entered and maintained in the electronic health record (EHR).
Clinical Support
Notify clinician promptly and act on priority conditions.
Set up and assist providers during exams and procedures.
Administer injections and support minor procedures, including specimen collection/testing, casting, splinting, and dressing applications.
Perform high-level disinfection of equipment and instruments.
Assist in maintaining medications/supply stock, emergency box, and oxygen supply. Maintain orderly work areas, records, and logs as required.
Patient Communication & Care Coordination
Provide discharge instructions and patient education.
Participate in motivational interviewing and self-management support, including goal setting and readiness-to-change techniques.
Serve as a point of contact for outside physician offices, pharmacies, transportation services, and medical facilities.
Assist with appointment scheduling and referrals to appropriate clinics.
Triage patient phone calls and provide in-basket (bucket) management support for clinicians.
Flexibility & Collaboration
Float as an LPN in other clinics as assigned to support patient care needs.
Maintain confidentiality of all personal health information.
Qualifications
High school diploma or GED.
Licensed as a practical nurse in accordance with Chapter 464, Florida Statutes, or eligible to practice nursing in accordance with Section 210, Florida Administrative Code.
Current Basic Life Support (BLS) Healthcare Provider Certification issued by a certifying agency, or the ability to obtain prior to hire.
Post-job offer, but prior to hire, the following will be required:
Documentation of current PPD (Purified Protein Derivative) test as performed by a qualified physician, reporting negative results within the past six months.
Documentation of physical examination completed by a qualified healthcare provider/physician within the past six months
Within 30 days of hire, the candidate must provide:
Documentation of Tetanus immunization within 10 years.
Documentation of Hepatitis B vaccine or positive antibody titer.
Preferred Qualifications
2-year degree/Associate's degree.
Experience with applicable computer software and programs, such as Microsoft Outlook, Word, Excel, and Electronic Health Records, or comparable programs.
Experience demonstrating effective customer service skills.
Experience communicating effectively verbally and in writing.
Experience establishing and maintaining effective working relationships.
Experience utilizing problem solving skills.
Experience maintaining accurate records and retrieving files from active/archive sources.
Knowledge of medical resources and terminology.
