Position Overview The Care Manager supports care coordination and utilization management for patients in inpatient rehabilitation. This role ensures appropriate level of care, facilitates transitions, and promotes efficient throughput while maintaining compliance with regulatory standards.

Responsibilities Essential Functions

Assess patients for medical necessity, care coordination, and discharge planning

Assign initial DRG and monitor length of stay using InterQual guidelines

Lead multidisciplinary care conferences and complex case reviews

Communicate with patients, families, and interdisciplinary teams regarding care plans

Ensure compliance with federal, state, and Joint Commission standards

Manage concurrent disputes and prevent avoidable days

Collaborate with physicians, nursing, and other departments to optimize care delivery

Qualifications

Education:

Graduate of an approved school of nursing

Licensure:

Current Florida RN license and BLS certification (or within 90 days of hire)

Experience:

Minimum 3 years in chronic disease management, care coordination, utilization management, or acute clinical care

Skills:

Strong communication, problem-solving, and organizational skills; ability to work in a fast-paced environment
