My Health My Resources of Tarrant County (MHMR) is looking for a skilled Service Coordinator. Each person receiving services in the HCS, TxHmL, and GR CFC in Tarrant County will have a single identified staff, called a Service Coordinator (SC). The SC will assist the person in identifying and articulating his/her priority outcomes, personal goals, and hopes for the future as well as his/her requests and need for services and supports. The SC will elicit the person's feedback, coordinate, monitor the services for desired outcomes as identified in the Person Directed Plan (PDP). The goal of a Service Coordinator is to provide the people we serve with seamless consumer driven services and support. The SC must also be knowledgeable about the array of services and supports available in the community.
Minimum Requirements Minimum Education:
Bachelor’s Degree in Health or Human Services
Acceptable Degrees Include:
Applied Behavior Analysis, Behavioral Services field, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services field, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special Education
Substitutions:
High School Diploma/GED and 2 years’ paid experience as a Case Manager in state Parent Case Manager Program OR Graduated from Partners in Policy Making and personal experience as immediate family member of an individual with an Intellectual and/or Developmental Disability
Required License:
Valid Texas driver’s license with an acceptable driving record
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
Preferences:
Bilingual a plus but not required
