Case Manager – Supportive Services for Veterans and Their Families (SSVF)

Position Overview The Case Manager will provide case management and supportive services to Veteran households for the Supportive Services for Veterans and their families (SSVF) program, including delivery of Rapid Resolution, Rapid Rehousing, and Homeless Prevention. The role involves strengths‑based housing case management services funded through VA SSVF, and pays $45,000 annually.
Essential Functions
Staff Training
Complete employee, HMIS (WellSky/CaseWorthy), Squares, ETO, and other trainings as required.
Attend in‑service training, VA training, supervisory meetings, and conferences.
Shadow other staff to learn the position process.
Screening Process
Conduct Rapid Rehousing and Homeless Prevention screenings within 24‑48 hours.
Document approvals, prioritizations, denials, and refer participants for supportive services.
Intake and Assessment Functions
Provide participant orientation and assess eligibility for veterans and mainstream benefits.
Obtain required enrollment documentation and conduct thorough interviews.
Maintain a participant file with all relevant documents and case notes.
Strength‑Based and Housing‑First Case Management
Deliver Rapid Resolution, Rapid Rehousing, Homeless Prevention, Health Care Navigation, Legal referrals, Financial Literacy, Life Skills, Employment services, and returning‑home support.
Address barriers such as mental illness, substance abuse, domestic violence, and probation status.
Document progress notes and conduct office/home visits.
Follow up at 30, 60, and 90 days post‑discharge.
Provide emergency/financial assistance to support housing stability.
Engage landlords and pursue affordable rental properties.
Housing Stability Case Planning
Assess and develop housing stability plans with the client.
Assist with budgeting and housing counseling.
Complete in‑home inspections and rent reasonableness forms.
Plan for self‑sufficiency and discharge outcomes.
Continuum of Care and Coordinated Entry
Implement COC community plans and prioritize by‑name lists.
Attend COC and Coordinated Entry meetings regularly.
Complete assessments for placement on the list.
Outreach, Engagement, and Community Networking
Build rapport with community partners and veterans.
Provide program information and educate the community on services.
Conduct outreach in rural counties and the 18‑county catchment area.
Secure meeting spaces and participate in outreach events.
HMIS/ETO Data Entry
Enter client data and outcomes into HMIS systems within 24‑48 hours.
Maintain accurate records of financial assistance.
Generate reports and maintain a 90% score on internal reviews.
Conduct peer reviews and other duties as assigned.
Qualifications
Basic Qualifications
Bachelor’s degree in human services (preferred Bachelor of Social Work) with 3‑5 years case management experience, or
Master’s degree in human services (preferred Master of Social Work) with 1‑2 years case management experience.
Or minimum five years relevant experience without a degree.
Knowledge, Skills, and Abilities
Excellent verbal and written communication.
Problem‑solving and conflict resolution skills.
Computer proficiency.
Experience working with diverse populations.
Time‑management and multi‑tasking.
Strong English documentation and communication.
Sound judgment and reasoning.
