Master's Degree Counseling or Social Work required

Experience

2 years of healthcare related Counseling or Social Work preferred

Licenses and Certifications

LMSW - Licensed Master Social Worker upon hire required

LCSW - Licensed Clinical Social Worker upon hire required

LPC - Licensed Professional Counselor upon hire required

LPC-I - Licensed Professional Counselor Intern upon hire required

LMFT - Licensed Marriage and Family Therapist upon hire required

LMFT-A - Licensed Marriage and Family Therapist Associate upon hire required

BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly required

CPI - Crisis Prevention Intervention Training upon hire required

ACPI - Advanced Crisis Prevention Intervention Training maintained annually 6 months required

CPI - Crisis Prevention Intervention Training 30 days required

ACPI - Advanced Crisis Prevention Intervention Training maintained annually 12 months required

What You Will Do

Provide individual, family, and group psychotherapy and counseling utilizing modalities consistent with the facility philosophy.

Conduct unstructured individual, family, and group therapy as scheduled or ordered by physician to address goals identified on patient treatment plan.

Provide didactic group therapy as scheduled and assigned.

Adapt treatment to reflect age-specific, developmental and cultural considerations.

Demonstrate competence with adolescent treatment.

Demonstrate competency with cognitive behavioral therapy.

Effectively provide individual, family, and group therapy in a timely manner meeting group therapy baseline competency elements.

Document therapies provided clearly, concisely and reflecting required level of clinical expertise.

Reflect patient goals for treatment and progress toward goals in documentation.

Plan, deliver and document groups in a timely and effective manner meeting patient goals for treatment.

Show knowledge of CBT and other appropriate brief therapies and interventions.

Demonstrate knowledge of age-specific and cultural-specific dynamics.

Participate in assessment of patients, conduct psychosocial assessments to determine patient and family functioning, implement effective treatment and plan appropriate discharge disposition, conduct PASR screenings as needed.

Maintain focus on patient safety at all levels of care.

Assess substance use and history to plan appropriate level of care, interventions and referrals.

Complete assessments demonstrating all baseline competency elements within required time frame.

Document psychosocial assessment completed.

Re-assess patient as indicated and document.

Integrate families and significant others in evaluation process with consent of patient as indicated.

Identify and document patient strengths.

Maintain professional accountability.

Maintain and enhance social work skills.

Serve as patient advocate.

Comply with personnel policies, e.g. Attendance Policy, Dress Code, Social Work Licensure Practice Act, etc.

Recognize and communicate ethical and legal concerns through established channels of communication.

Provide and accept constructive feedback in a calm, respectful manner.

Maintain licensure.

Complete CPI, HIPAA, BLS/CPR, 8 hours intake training (if applicable), and continuing education required for license annually.

Take action to protect patients rights and/or preferences, promote patient desired outcomes.

Demonstrate and document patient advocacy role when appropriate and compliance evident.

Treat ethical and legal concerns through appropriate levels of supervision.

Participate as a treatment team member.

Coordinate patient care with other disciplines.

Build professional relationships with peers, customers, and supervisors.

Offer assistance, preceptorship, and supervision to other staff and students.

Maintain rapport and confidence of physicians.

Adapt to changes in workload and shift assignments to meet changing needs of the patients or programs.

Maintain productivity.

Demonstrate positive, constructive communications.

Ensure patient care consistent and coordinated between disciplines.

Mantain rapport and confidence of peers, customers, and supervisors.

Assist and mentor peers and students willingly.

Provide oversight for assigned students and monitor performance.

Ensure physicians express confidence and rapport with staff member.

Provide discharge planning consistent with patient level of functioning and biopsychosocial needs.

Assist and educate patient and families with identifying appropriate transition plans.

Provide information on resources available based on identified patient needs.

Collaborate with treatment team and physician to establish and facilitate discharge plan.

Coordinate referrals and transfers with providers, community agencies, EAPs, payor source, and staff.

Obtain discharge plan orders as indicated.

Document transition as appropriate for program.

Ensure transition planning appropriate for each individual patient.

Document transition plan and resources.

Make appropriate referrals/appointments based on identified patient needs.

Provide knowledge of community resources.

Communicate with treatment team and physician.

Assist with development and documentation of comprehensive treatment plans for each patient.

Identify realistic, appropriate, and individualized goals for patient care.

Plan care to meet treatment plan goals.

Produce documentation supporting the treatment plan and meeting regulatory requirements.

Coordinate care with treatment team and patient.

Orient patients to programs and unit guidelines.

Document individualized treatment plan goals, initial assessment, and reviews.

Add and update goals as patient progresses.

Use goal directed interventions.

Maintain documentation accuracy, completeness, legibility.

Attend and participate in assigned Treatment Team meetings.

Participate in providing effective and cost efficient quality program services.

Work to continuously improve quality of service programs.

Maintain accurate and complete billing and accounting records.

Understand regulations and accreditation standards.

Maintain and enhance knowledge and skill in psychiatric and chemical dependency treatment.

Complete process improvement projects when asked.

Acknowledge Key Performance Indicators and Key Initiatives, take actions to meet requirements.

Document attendance or review minutes of all inter-disciplinary Staff, PASR and other required meetings.

Exhibit appropriate utilization of resources.

Meet or exceed expectations of customers including patients, families, EAPs, psychiatrists, PCPs, external case managers, referral sources, and receiving resources.
