Hospice Palliative Social Worker – MSW II
Location: 8100 Constitution Pl NE, Albuquerque, New Mexico 87110-7643, United States
Compensation: $27.92/hr – $47.53/hr (Base pay range for this position).
Job Responsibilities
Develop, plan, implement and evaluate psycho/social interventions for patients in Home Health, Hospice IP/OP and Palliative programs.
Identify patients with complex psycho/social needs and assist with eligibility determination for social programs and community assistance.
Serve as the mental health professional of the interdisciplinary team and provide crisis intervention to patients and families.
Coordinate and facilitate development of the Plan/Goals of care for all patient populations served by home care teams.
Receive referrals and consultations for patients from at‑risk populations and interdisciplinary team members; deliver interventions in person, via phone, or telehealth.
Qualifications
Master’s degree in social work required.
Current license from the State of New Mexico as a Licensed Independent Social Worker (LISW), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Work (LMSW).
License with hospital social work or discharge planning experience required.
Licensed Master Social Work License (LMSW) preferred.
National Case Management certification preferred.
Current driver’s license and insurance required.
