Position Summary The Director of Social Work (LMSW) leads the Social Services department, ensuring residents receive high‑quality, person‑centered care. The role supervises social work staff, coordinates with interdisciplinary teams, manages compliance and documentation, and advocates for residents and families.

Primary Responsibilities

Supervise and mentor social work staff.

Establish department goals, policies, and workflows.

Promote best practices and maintain a positive work environment.

Assess residents’ social, emotional, and adjustment needs.

Develop and update individualized care plans.

Provide counseling and guidance during transitions.

Collaborate with nursing, medical, therapy, and administrative teams.

Communicate regularly with residents’ families regarding care and progress.

Participate in interdisciplinary care plan meetings.

Ensure adherence to state and federal long‑term care regulations.

Oversee documentation, assessments, and required reporting.

Maintain confidentiality and uphold ethical standards.

Connect residents and families with local community resources.

Represent the facility in outreach or agency interactions as needed.

Qualifications

Licensed Master Social Worker (LMSW) in New York.

Experience in long‑term care required.

Prior leadership or supervisory experience preferred.

Strong communication, problem‑solving, and organizational skills.

Knowledge of long‑term care regulatory requirements.

Ability to manage multiple priorities effectively.
