Performs intermediate skilled human support work in a criminal justice setting. Work includes assessing individual needs and providing access to needed and appropriate services including treatment, linkage to rehabilitation services, case monitoring and consumer advocacy functions, and related work as apparent or assigned. Work is performed at Riverside Regional Jail in Prince George, VA under the limited supervision of the assigned Program Supervisor.
Job Duties
Provides support through the transportation-encouragement-role model. Facilitates group meetings; composes minutes for meetings. Develops wellness recovery action plans with consumers; develops a plan of care appropriate to the evaluation with the consumer/family consensus; assists consumers in developing recovery goals. Monitors and assesses consumer’s mental health status; provides supportive counseling as needed. Provides crisis intervention services. Arranges and provides transportation for consumers to appointments, grocery stores, offices, etc; takes consumers on social outings. Records all services in the EHR within the established time frame set forth by policy and standards. Develops staff and consumer schedules. Attends training and team meetings as required.
