Responsibilities
Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, and follow‑up, documenting all findings according to Matrix guidelines.
Administer and order point‑of‑care and lab‑based testing as needed.
Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
Work with PCPs on patient education and follow‑up.
Provide services at the member’s place of residence, including homes and skilled nursing facilities.
Benefits
Flexible scheduling: visits are available Sunday–Saturday, 8 a.m.–7 p.m.; work day, evening, or weekend according to your availability.
Competitive pay: earn $90–$115 per assessment; those completing 20–100 visits per month can earn $1,800–$11,500.
Benefits: mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
Education and support: comprehensive onboarding, 1:1 feedback, free continuing education.
Licensure support: assistance with additional state licensure and credentialing as needed.
Full‑time opportunities: PRN providers can transition to full‑time roles with a sign‑on bonus.
Qualifications / Skills
Master’s degree or equivalent experience and NP licensure.
Current NP licensure in state of practice, including prescription authority or ability to obtain it.
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
Current BLS, ACLS, or CPR certification.
Minimum of 1 year NP experience (new grads encouraged).
Valid state driver’s license and proof of adequate automobile insurance.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
