Job Title and Location
Nurse Practitioner – PRN (HealthFair) – East Chicago, IN. Salary range: $1,800–$11,500 per month. Sign‑on bonus: $1,500.

Responsibilities During a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) improves quality of care and closes multiple care gaps in a single visit.
Conduct adult/geriatric assessments: medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation per Matrix guidelines.
Administer and order point‑of‑care and laboratory testing as needed.
Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.
Provide services at the member’s place of residence, which can be homes or skilled nursing facilities.
Benefits
Flexibility: visits scheduled according to your availability from Sunday–Saturday, 8 am–7 pm.
Competitive pay: $90–$115 per assessment; 20–100 visits per month can yield $1,800–$11,500.
Mileage reimbursement, 401(k), referral bonuses, and voluntary benefits.
Education and support: onboarding, preceptor, 1:1 feedback, free continuing‑education access.
Licensure support: assistance with additional state licensure and credentialing.
Full‑time opportunities: PRN providers may transition to full‑time roles with a sign‑on bonus.
Qualifications
Master’s degree or equivalent experience and NP licensure.
Current NP licensure in state of practice, including prescription authority.
Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
Preferred 1 year NP experience; new grads encouraged.
Valid driver’s license and adequate automobile insurance.
Strong computer skills and familiarity with medical record software.
Excellent verbal and written communication skills.
